Old Students Enrollment Procedure

A.Y. 2023 – 2024 , 1st Semester

STEP PROCESS ONLINE IN PERSON
1

Application and Permit to Enroll

Accomplish the application form via google form link:

NOTE: Irregular Students must download the Pre- Enrollment form using the link below before proceeding with the application form and permit application link. Make sure you are cleared in all departments and your grades are all complete. Otherwise you will need to submit the signed agreement forms (Scholarship is applicable and registrar form)

Assessment / Report Card Printing

The registrar will process your assessment and will forward to Finance.

Make sure you re cleared in all departments and your grades are all complete before you proceed to the Registrar Window at the Community Center to get your report card and PEP for irregular students. Claim your Assessment.

Grades Validation

ATC to validate grades.

Submit the original and photocopies of grades and Assessment of Fees to the Grades Validation Counter

Verification of Fees

Finance to verify the fees.

Proceed to the verification counter at the Community Center.

Scholarship and

Grant Application Processing

Finance will email the verified Assessment, Promissory Note and Grant Disclosure to the students.

Students to email the following ( Original Documents ):

1. Scanned copy of Student ID with 3 Specimen Signatures.

2. Scanned copy of Parent/Guardian with 3 Specimen Signatures.

3. Promissory Note form (For all with grants and scholarships

except for EBA, EPS, DOST, TES)

For those who will not be able to come personally to the school to submit and sign the documents, you may have it signed by your Department Chair/Dean /Faculty Guardian.

Send the following documents below via courier:

1. Authorization Letter authorizing your faculty guardian to process your enrollment  (Originally signed)

2. Photocopy of Student ID with 3 Specimen Signatures

(Originally signed)

3. Photocopy of 3 Specimen Signatures of your

Parent/Guardian (Originally signed)

* We will only reflect your scholarship / grant in the

Statement of Account if these requirements are complete.

Students will fill out the Promissory Note and Grant Disclosure and submit the following:

1. Photocopy of Student ID with 3 Specimen Signatures.

2. Photocopy of Parent/Guardian with 3 Specimen Signatures.

3. Originally signed by students and parents Promissory Note form (For all with grants and scholarships except for EBA, EPS, DOST, TES)

Uniform Measurement and Placement of Orders

Have your uniform measurement and placement of order done once you are available to go to school.

Have your uniform measurement and placement of order at the bookstore

Payment of Fees and Printing of Statement of Account (SOA)

You may choose from any of the available payment gateways.

Finance will prepare your Statement of Account. You will receive the scanned copies of your COM and SOA in the email address you have provided.

Pay your tuition and other fees at the FAITH Business office

/ cashier Windows 1 and 3 or to any accredited payment centers.

Get your SOA at the Business Officer/Cashier Window 2

Printing of

Certificate of Matriculation

(COM)

Registrar will print your COM and email you the copy of your COM.

Proceed to the Registrar Office Window 1 for the Printing of your (COM)

ID Update and Printing

Upload ID picture and signature in the student portal.

Guidelines of photo and signature can be found in your portal.

Proceed to NSC Room 111 for ID Capture and Printing. Please bring your own uniform.

Note: Online enrollees may claim their ID and hard copies of COM at the Registrar’s Office 

and their Official Receipts and SOA at the Business Office/Cashier.
