- ESC Certificate (if applicable)
- For SHS Transferees
• Grade 10 and 11 Copy of Report Cards
If the above credentials are not immediately available, you may submit one of the following documents so you can proceed with the enrollment:
- SY 2025-2026 Report Card covering the 1st to 3rd Grading Periods
- Certificate of Completion/Graduation in lieu of Form 9/Form 138
